Incident report form does not get added to the session causing the submit report to fail

      The incident report form does not get added on the session. The UifAnnotationMethodHandleAdapter does not get called for the incident report which causes the UifServletRequestDataBinder.bind not being run for the form. This causes the form to be null in controller methods (ie. 'cancel', 'submit report') being called after the view has been rendered.
