Action Requests re-appearing after being approved after a rule re-resolve of a document

      Description

      Essentially what occurred here is that a user added about 20 rules a few minutes apart which caused enroute TK documents to re-resolve. When they did this, approvals that some people had already taken were showing back up in their action list. This may not be easy to reproduce but we will at least want to look into it and attempt to reproduce in a unit test.
