          Talked to Jerry about this, the sessionId hidden on the form should be populated with a value, currently the source of the IncidentReport page has a hidden that looks like:

          <input type="hidden" name="sessionId" value="" >

          which is what is causing the snippet of code in my previous comment to evaluate in a problematic manner.

          Steve was able to find a resolution. I am re-assigning so that he gets credit. Nice job Steve, and Corey.

          Steve,

          I believe there are still some issues with this button. Also, I question whether the cancel button is correct in this case. There is no 'action' to cancel (a document was not initiated). I think the buttons we would want to show are close (which would close the window), and possibly back (to go back to doc search).
